Membership Terms & Conditions:

      • All memberships have a 6-month commitment and a $25 one-time Administration Fee

      • Failure to make a payment will result in a suspension of all membership benefits until the account is brought current.

      • Not all services are recommended for monthly use. Members should adhere to recommended usage frequencies to ensure the best results and maintain overall skin health.

      • Monumental Med Spa enforces a non-refundable policy for membership payments. Once a monthly payment is made, it is acknowledged as final.

      • Gift cards, coupons, and points are not permissible forms of payment for monthly membership fees.

      • Memberships and credits are non-transferrable and cannot be shared or split between individuals.

      • Members must redeem their entitled services within the corresponding month of their membership period. Any unused services or benefits will not be carried over or accumulated into subsequent months.

      • Payments are automatically billed monthly

      • At times Massage may be unavailable

      • To cancel the membership a Membership Cancellation Form must be completed online or in our office.

      • Additional terms and conditions apply

      • Appointments Cancelled less than 24 hours will still incur a $50 cancellation

      • A full Membership Terms & Conditions Form must be signed at time of purchase
